CVD Prevention
About the programme
Cardiovascular disease (CVD) is the leading cause of death worldwide, with hypertension, followed by hypercholesterolemia CVD is strongly associated with health inequalities (the most deprived quintile of the population is 4 times more likely to die from CVD than the least deprived).
The NHS Long Term Plan (LTP) states that the biggest area where we can save lives is in reducing the incidence of CVD. CVD causes a quarter of all deaths in the UK and is the largest cause of premature mortality in deprived areas. Hypertension (HT) and Hypercholesterolaemia are significant risk factors for CVD, and evidence suggests that improving the detection and treatment of those with HT and hypercholesterolaemia will have significant benefits. Health Innovation West Midlands (HIWM) will support systems to case find, risk stratify and optimise individuals and ensure treatment to target and improve patient access to appropriate medicines to enable lipid treatment using the National Guidance for Lipid Management, specifically in Primary Care Networks (PCNs) with the highest level of inequalities, and ensure pathways are in place that allow for the identification and subsequent management of people with Familial Hypercholesterolaemia (FH).
Our Approach
HIWM is taking part in the CVD Portfolio approach as part of the wider Health Innovation Network. Running from 2024 – 2027 this will involve engaging and supporting the adoption of the various risk stratification tools available and holding education and training sessions in order to support in the identification and optimisation of patients.
This will cover Hypercholesterolaemia -including FH and Blood Pressure Optimisation (BPO).
Programme ambitions
- Improve prognosis of those living with heart disease to ensure they are optimally managed to reduce number of heart attacks and strokes (by diagnosing and treating patients with high cholesterol, high blood pressure and FH)
- Adopt a population health management approach to addressing CVD prevention and management
- Address health inequalities
- Ensure equitable access to evidence-based and/or NICE approved treatments (medicines, devices, medical technologies)
- To identify, diagnose and manage patients with FH
